|Description:||LSI seeks a full-time Care Coordinator for the Family, Safety, Risk, and Permanency (FSRP) services in Black Hawk county. |
|Duties:||Provides direct service to assigned clients through use of assessment and diagnostic skills, varying intervention modalities, crisis intervention, case management, and advocacy by traveling by car to their homes, an office or other community settings, or by telephone. These activities may occur during normal business hours, after hours, or on weekends. |
Coordinates and works collaboratively with the Family Support Worker to provide safety and permanency services to assure service delivery requirements are met for each case. Also monitors and effectively manages service delivery outcomes for achievement on each case, provides clinical support to Family Support Workers.
|Qualifications:||Preferred applicants will have a Bachelorís degree in human services or a related field from an accredited four-year college with a minimum of two (2) years full-time experience in Child Welfare Services or, a Masterís degree in human services or related field from an accredited college or university. At minimum, applicants are required to have Associateís of Arts degree plus four years full-time experience in Child Welfare Services.|
This position requires a valid Driver's License, Auto Insurance, a good driving record and the ability to pass an extensive background check including checks of the Dependent Adult and Child abuse registries, Sex Offender registry and a criminal history check.
